DeMarcus Cousins believes Joel Embiid’s long-term health depends on significant weight loss. Speaking on FanDuel TV’s Run It Back, the former All-Star center gave blunt advice to Embiid.

“For me, it’s the weight. You’re at the point now in your career where your weight matters,” Cousins said, via HoopsHype. “You’ve been through injuries, you’ve had surgeries, and obviously, it’s still a nagging issue.”

Cousins emphasized the toll Embiid’s size takes on his body.

“Now, it’s about holding yourself accountable—go lose 40 pounds. Obviously, your body can’t sustain that type of weight,” he continued. “The wear and tear, the constant pounding on your knees night after night—it’s not going to help your situation.”

The four-time All-Star insisted that the modern NBA does not require big men to carry as much bulk as before.

“This is a rail-thin league. It’s about moving up and down, side to side. It’s not about physicality and bruising,” Cousins added. “So, go lose 40.”

Embiid’s season remains in jeopardy due to persistent knee issues. Philadelphia 76ers head coach Nick Nurse confirmed that the team has not ruled him out for the year, but his status remains uncertain.

NBA insider Shams Charania reported that Embiid has been consulting with doctors for alternative treatment options. His current regimen, including injections, has failed to show significant progress.

The former MVP’s left knee has been swelling for weeks, raising concerns about his ability to return this season.

Embiid has played just 19 games in the 2024-25 campaign, and his production has dipped significantly. For the first time in nearly four years, he scored 15 points or fewer in back-to-back games.

His season averages stand at 23.8 points, 8.2 rebounds, and 4.5 assists per game, well below his usual numbers.

The 76ers entered the season with championship aspirations after acquiring Paul George, but their playoff hopes have faded.

Philadelphia currently holds a 20-37 record, sitting 12th in the Eastern Conference.

The team now faces tough decisions regarding Embiid’s recovery and whether to shut him down for the remainder of the season.
